How do I change my beneficiary?
The policy owner may request a change of beneficiary at any time by completing the BENEFICIARY section of our change form. Date and sign the form before a witness who has no interest in the policy and return it to the Company at the address shown at the top of the form. The beneficiary change will be made when the completed form is received, and a copy will be returned to you for your records.

The current owner may request an ownership change by completing the OWNER section of our change form. The current owner must date and sign the form before a witness who has no interest in the policy, and return it to the Company at the address shown at the top of the form. The ownership change will be made when the completed form is received, and a copy will be returned to you for your records.
NOTE: Please consult your tax advisor before requesting an ownership change, as this change may result in adverse tax consequences for you.

If the Insured, Owner, Payor or Beneficiary's name has changed due to marriage or divorce, or if a change needs to be made to correct our records, the owner may complete the NAME section of our change form. The owner should date and sign the completed form before a witness who has no interest in the policy, then return it to the Company at the address shown on the form. The name change will be made when the request form is received, and a copy will be returned for your records.

Can I pay premiums automatically from my bank account?
Yes. While your policy is in force, you may
request premiums be paid automatically from your checking or savings account.
Please FAX or mail our completed request form to the Company with a voided check
or deposit slip from your bank account. Our FAX number and mailing address are
shown at the top of the request form.

If you need to file a claim for death, long-term care, or waiver of premium benefits, you may write, email, FAX or call our Policyowners' Service Department. Please furnish the Insured's name, social security number, policy number(s) and date of death, if applicable. Please also give us your name, mailing address and phone number. If your policy has lapsed, you may request
reinstatement by completing our Application for Reinstatement. Please answer
all questions in detail, sign the form in both places, and return it to us with
your past-due premium and/or loan interest payment. Your request for reinstatement
will be given our prompt consideration.
If you received a letter from us within the last ten days, you may use the reinstatement
amount quoted in that letter. If it has been longer than ten days, please call
our Policyowners' Service Department to confirm the amount you need to remit.

How can I get financial information about my policy?
You may write or call our Policyowners' Service
Department for financial information about your policy. However, because your
privacy is very important to us, we do not release financial information over
the phone or by return email. We will gladly mail a current statement or other

How do I request periodic interest withdrawals from my annuity?
You may request automatic interest withdrawals
from your annuity by completing sections 1, 3 & 4 of our request form. Automatic
withdrawals can be made monthly, quarterly, semi-annually or annually. If you
choose the convenience of automatic bank deposit, be sure to furnish a voided
check or deposit slip when you return the completed request form to the Company.

How do I request a one-time partial withdrawal from my annuity?
Partial withdrawals from the cash accumulation
value of your annuity are normally processed within two business days. The owner
must complete sections 2, 3 & 4 of our request form and return it to the Company.

a voided check or deposit slip with your completed request form. You may send

How do I request Required Minimum Distributions from my IRA?
Minimum Required Distributions from your IRA
can be made automatically. If you wish to have your distributions deposited
into your bank account, please include a voided check or deposit slip when you

I have lost my policy. How do I request a duplicate?
If your policy has been lost or destroyed,
you may request a duplicate by completing our Duplicate Policy request form.
This form must be signed by the policy owner in the presence of a Notary Public.
A duplicate policy will be issued when the notarized form is received at the
Home Office of the Company. Our mailing address is shown at the top of the request
form.
